University personnel can review this procedure and submit a DEHS 'Equipment Release' form via DEHS website. The Principal Investigator is responsible for ensuring that DEHS is contacted prior to movement or service of laboratory equipment for the purpose of verifying and documenting that the appropriate decontamination has been completed. Laboratory equipment, such as fume hood, refrigerator, freezer, centrifuge, autoclave, etc., that may be contaminated with biological, radiological and/or chemical hazards must be properly decontaminated prior to service or movement by Physical Plant personnel from the laboratory for surplus, repair, disposal, or relocation.
